I had the pleasure of meeting Adriana today and she really went above and beyond to ensure that my father had all the resources that he needed upon discharge and that he was sent home safely. She was able to arrange medical transport to his home and also back and forth to dialysis until he is strong enough to require less assistance. She provided me resources to get connected with a very responsive caregiver service, coordinated his home health needs, and even coordinated his physical therapy at my father house.
She not only was caring towards my father but my mother as well. She was extremely patient with her and patiently and thoroughly explained my father's health needs and services being provided and reassured her and myself that my father will be receiving the care he needed at home. She is compassionate and reassuring and provided a sense of security that my father will be well caredfor at home. She consistently communicated to me kept while coordinating his discharge needs; even when I was not at his bedside.
I would like to recognize Adriana for all her efforts; her care and dedication as it brought much comfort to myself, my mother knowing that my father was being sent home with all his needs cared for. Adriana has really been exceptional.
