Angel went out of her way to help her unit and a patient at his end of life. The patient had terminal diagnosis and had been extubated per family's request in ICU and sent back to 5E (medical oncology) for end of life care. Angel had taken care of him prior to his transfer to ICU and was also communicating with the family about end of life care and concerns. On the evening the patient was to be discharged to hospice the ambulance was to arrive at 1700. At shift change, Angel agreed to stay until the ambulance arrived as she had made a promise to the attending physician that the patient would receive a very specific dosage of medications in order to comfortably make the trip to the hospice facility. Staffing for nights was such that this patient was not included in census as he was supposed to be gone by 1700. Over the course of the evening, several calls were made and yet no ambulance arrived despite repeated promises and times. Finally at 2256, the ambulance arrived to transport the patient, nearly 6 full hours after their promised arrival time. Angel stayed to make sure the patient received the prescribed care, and while she and I discussed this over the phone, she mentioned to me that she was supposed to start her vacation at the (usual) end of her shift. Not only did she stay over, she fulfilled her responsibility to the patient, her promise to the physician, and postponed her vacation in order to do so.
