Brittany went above and beyond to advocate for her patient. She received a patient that is well known to the SM Med/Tele team for frequent visits. This particular patient has been deteriorating for months with a poor prognosis. Yet a safe and manageable plan of care was hard to achieve with the patient's POA, as she was not ready to consult palliative. Brittany personally spent hours with this patient's POA, connecting with her and sharing personal life events with her and connecting with her as an individual and not just as her mother's nurse. She made the POA feel listened to and showed that she truly understood and cared for the patient and for the POA's situation. By the end of the conversation, the POA was finally in agreement to begin to discuss palliative care.
