I am 100% sure that if Brooke had not advocated for this patient as she does all of her patients, he would have likely not survived.
In Transitional care, most of what we do is by Telephone. It makes it difficult sometimes to engage patients and get them to do what they need to do to be healthy. Brooke demonstrates day after day an excellent example of going above and beyond to assist others. She is the first to offer to manage her co-workers' patients when they need to take time off. She has stepped up to work at the COVID Clinic when our COVID RN started her inpatient new grad position, and she is an amazing support to myself and the transitional care team. These are all great reasons why she is a DAISY Nurse, but specifically, I truly believe she saved the life of one of our COVID patients. This very difficult, homeless patient was discharged from the hospital after a long inpatient stay requiring oxygen. He was discharged to isolate himself in a motel under county housing. Brooke set him up with a follow-up appointment in our COVID Clinic for 3 days after discharge. She made her first follow-up call to the patient on day 1 after discharge, he was short of breath and said he could not figure out how to use the oxygen machine. She talked him through this and he was able to breathe a little better. He was febrile and tired but stated he was ok. She called him 2 days later to remind him of his appointment that afternoon and he adamantly refused and said he did not want to go, he just wanted to sleep. Brooke tried to explain to him that it was really important that he attend the appointment. He continued to refuse. She could tell he was groggy and SOB. Brooke proceeded to call Freedom Medical Transport who was providing transportation for the patient and instructed them to proceed with going to his location and not to take no for an answer...insisting that he come to his follow-up appointment. The patient showed up to the appointment and was completely ashen (gray) and SOB on the 4L Oxygen. Clinic RN and MD quickly assessed the patient and arranged a direct admit to the COVID unit. I am 100% sure that if Brooke had not advocated for this patient as she does all of her patients, he would have likely not survived. Brooke is an amazing Nurse and deserves to be recognized for all that she does for our community of patients. We are so blessed to have her on our team. Thank you!
