Grace is an amazing nurse. One of Grace's strengths is the compassionate care she provides to all her patients. She is also a strong patient advocate, especially for those who are unable to do so themselves. I would like to share a recent patient story that exemplifies these characteristics of Grace's nursing practice. Recently, Grace was caring for a patient who was admitted to the hospital with a non-surgical dx that could be treated with oral antibiotics. The patient was homeless and was scheduled to be discharged. Grace was concerned about the discharge plan. She contacted the medical team to express her concerns that the patient was exhibiting physiological signs of withdrawal and his desire to seek treatment. She was able to collaborate with the medical team, social worker, and case manager to provide the patient with information and bus transportation to the nearest rehab. An hour after the patient was discharged, the rehab center called us to let us know the patient made it to the center. Grace connected with the patient and recognized his needs above all else. It was great to see how the team collaborated together and the positive outcome the patient experienced.
