Jac is amazing. She was the case manager for my mom's in-home health care while my mom and dad were visiting their summer home. She made an instant connection with my mom and with my dad and me on the very first visit. She made it known that we could call her anytime with issues or problems. The very next morning, my mom called her and said she had a fever. Jac got there as quickly as she could, rearranging her day to accommodate my mom. Jac kept in touch, monitoring how she was doing over the next week. After another bout with fever, my family made the decision to move my mom back to the care facility where she resides. I called Jac for advice on the best way to go about getting my mom into the skilled nursing that she temporarily needed. She was in the area and immediately came over and assessed my mom and made sure she was ok to travel the 2.5 hours back to Maryland to be admitted to the hospital closest to where my parents live. She stayed in contact with me throughout the day to make sure my mom was doing okay. This is so much more than a job for Jac. She has such a passion for what she does and the people she cares for. She made a very trying and stressful time manageable with her personality and her passion.
