Lisa knows all of her home visit patients and their families by name and they are comfortable with her. She advocates for their special needs as home and bed bound patients and speaks up for those who are not able to speak for themselves. Lisa makes home visits to our most frail elderly patients and must be relied upon to be very trustworthy as she makes her visits alone. She has gained the trust of her 2 MDs, the NP, social worker, chaplain, manager and most importantly her patients and caregivers. She has never made an error in her work drawing blood and giving vaccines but she never boasts or expects any recognition for this fairly incredible statistic.
Lisa is respectful to staff and patients regardless of how others treat her. She is quite cheerful in her approach and patients and caregivers respond in kind by being very comfortable with her. Lisa is the "glue" of the program; she helps manage all phone calls, paperwork and home visit schedules for clinical staff who are all out making visits each day. She is an active member of our interdisciplinary case conference who contributes a great deal of valuable information about all patients/families.
Lisa is willing to "pitch in" and help out regardless of her schedule and helps cover Geriatrics Clinic as well as House Calls if a nurse coworker is out. Lisa is very proud of the HouseCalls Program and we are very proud of her incredible record of accuracy in a very challenging position of serving frail elders in their homes. Lisa is always positive and a great role model and she takes pride in serving patients regarding of their home situation. She is careful with all supply and medication orders. She schedules visits by neighborhood to reduce her driving and thus decrease mileage costs and stress.
Lisa is very deserving of the DAISY Award because she has helped organize all the home health care paperwork for the program in support of 2 MDs and 1 NP; given the acuity of HouseCalls patients and the complicated process for certifying any home service need, this is a huge and challenging amount of work that she manages so well for the benefit of the entire program.
