This group of our rehab staff has been taking care of a patient we have who is quadriplegic and trached with a vent after a traumatic accident. He has been alone for most of his 3-month stay here and will be transferring to a hospital in Tennessee instead of going home due to a lack of caregiver presence. This group of wonderful people went above and beyond to make a "How to Care for Me" binder for the patient. It goes into detail for both OT and PT specialties, describing their daily tasks with him and certain things to be aware of in his care. The nurses added a whole detailed section about the care we have been doing for him including his likes/dislikes with his care. The patient can verbalize appropriately but is unable to actually do anything for himself, so they wrote out things to know about him in hopes that those taking care of him in the future have a head start. This whole team spent hours organizing and compiling this binder so that the next hospital and eventually his family is set up for success when caring for him.
