Staci is an experienced nurse in community health and is an excellent role model on our team. She is very compassionate with our patients and will always go above and beyond to do what is right for the patient and family and is very resourceful. She is well respected by our patients, their families and her peers. She recently submitted a research article to the AAACN regarding blood pressure accuracy in the adult primary care setting. She cares deeply for our patients and has a high standard of nursing care that she practices daily. She is consistently a first "good impression" with our patients and families. She is warm, welcoming, caring, and greets them with a smile and is the same way when they have completed their visits. They look forward to coming back and seeing her in PCC and often call the clinic specifically asking for her. It is evident that she has established a trusting relationship with them. She is the Lead nurse in our Wednesday evening clinic. She ensures that the "loop" is closed with patient issues and is a true advocate for these patients.

Staci has made numerous connections with our patients and families in PCC. There is one patient in particular, who works a full-time day job and had been utilizing both the YNHH ER and Foxon Rd urgent care clinics for his chronic health care issues and had no Primary Care doctor. Staci was instrumental in getting him established in the Wednesday evening clinic so that it did not jeopardize his day job (for fear he would lose his job by taking time off to go to doctors' appointments). Staci established a trusting relationship with this patient who has been keeping his Wednesday Evening Clinic appointments, has not been utilizing the Emergency Room or off site urgent visit clinic for his primary care needs. He has been very engaged with his care and happy with the care he has been receiving. Staci has developed a nurse-patient partnership that has made a positive impact on this patient's experience and has made a positive impact on his current health management.

Staci has worked very hard with our patients and staff a lead nurse with her extensive wound care knowledge and experience. Staci will often work closely with our most complex patients with wound care issues. She is a resource for both nursing and physicians when it comes to advice on wound management. Staci works very hard to provide patient education with our complex wound care patients. She collaborates closely with our VNA nurses and providers on the plan of care managing chronic wounds in the outpatient setting. She has prevented many of our patients from being readmitted and having multiple ER visits by her timeless education, dedication and attention to detail she has provided to these patients. She is knowledgeable in wound care resources and products that are most current and up to date.
I can think of one particular patient who has had multiple readmissions to the hospital for chronic lower leg wound care infections. Staci spent many hours educating him on how to self- manage at home and worked closely with his provider and homecare nurses. He actually walked into clinic recently without using a wheelchair and his wounds were almost healed! Her time and dedication working with this patient has shown reductions in ER visits and readmissions.
Staci McNeil is well liked and respected by her patients and peers. She demonstrates professionalism, positive energy, dedication, and commitment to our patients and staff. She cares deeply for our patients and is compassionate and caring. She strives to make our unit a better, safer place and is our HRO unit leader. She understands the importance of patient safety, encourages us to utilize CHAMP, and reminds us that we are all 100% accountable in our practice.
