Amanda consistently displays excellent critical thinking skills and adaptability in her work. When identifying potential discharge barriers, she is always an active participant in finding a solution, many times presenting these issues to case management/social work with an appropriate solution.
Recently a difficult to place patient was to be discharged late in the day after the case manager/social worker had left for the day. This was a challenging patient in many aspects. I had left for the day with everything arranged for the patient to discharge to what she had reported being her facility of choice. Immediately prior to transportation taking the patient to this facility, the patient refused to be admitted to this facility. It was at this time that the patient selected a different facility. I was made aware of this change, as well as the fact that Amanda had already notified the newly chosen facility.
The patient had previously chosen this facility so there had already been a referral sent and the patient ended up being discharged from the hospital later the same evening. This prevented further avoidable delays due to the patient's initial refusal and recurrent vacillation in the discharge location. Amanda was also precepting a new RN as another responsibility she managed that day. Her actions not only assisted FHMC to effectively care for all of our patients by making that bed available but also set a stellar example for a new nurse.
