Anna has anticipated their decline and participated in educated discussions with the specialist palliative care team about how she can best provide high-level care to this patient with complex care needs.
I am a CNC for SPCS at the RHH. We acknowledge that for many acute care medical staff palliative and EOLC can be confronting and difficult to manage personally but also difficult to understand the signs of deterioration.
Over recent weeks I have recognised such intuition and understanding from Anna in regards to the palliative patient’s she has cared for. In particular, one this week.
Anna is a graduate registered nurse who is only six months into her program. She has shown a deep understanding and engagement with this patient’s clinical deterioration. She has anticipated their decline and participated in educated discussions with the specialist palliative care team about how she can best provide high-level care to this patient with complex care needs.
Anna then acted swiftly, implementing critical changes in this patient’s care to maximise their comfort. She has responded accurately to a range of symptoms and managed to escalate care when required. All whilst looking after multiple other complex patients on an acute ward.
This patient has a very large and loving family. They have been in with this patient since admission and at times emotions run very high. Anna has supported the patient and their loved ones with such empathy and knowledge far beyond her years. This family has a significant grief history, and Anna has eloquently supported them through this difficult time. She has supported this patient and their unique needs to facilitate their final wishes and assisted the family in grieving, all whilst providing high-level care.
Anna has exceeded all expectations and shows exemplary signs of compassion and dedication to her work and her patients.
