Camille was able to help build rapport and trust with the family as they were new to this country and learning this medical system.
Camille, a nurse care manager, has worked with a patient and family that recently came to the US as refugees. The patient has a number of needs, and the family, in addition to adjusting to a new country, are trying to learn the medical system and make sure the patient's needs are met. Camille has spent many hours through two separate admissions working with the patient's dad, the community agency helping the family, home health, and others to help provide the best care and support for the family. As the community agency involved with the family continues to learn about the medical process, she has been a point person and advocate for the family. She has worked closely with the social work care manager and has done things, including helping set up a care conference to develop further processes to support the patient and family.
Camille was able to help build rapport and trust with the family as they were new to this country and learning this medical system. She worked tirelessly to set up things for discharge, including home health, which was done with some difficulty. Through the care conference, she was part of developing a further plan for the patient and family and did many things following that to help with the discharge process.
