There is never a dull moment when you are part of our DH team, and we are all proud and supportive of each other.
We all have heard the sayings like “teamwork makes the dream work” or “there is no I in TEAM,” and those mindsets are precisely what we drive through in the DH team. When our UBC kickstarted from the third wave of RBC, we never forgot the heart and purpose of UBC: and that is to improve patient outcomes with City of Hope’s mission and values at the core of it, with our hearts of compassion behind it, and with the spirit of collaboration as the driving force behind it.
Care of Patients and Families: Within one year, we have started patient-centered initiatives like enhancing the auto transplant patient experience, encompassing a welcome tent card, a day “0” transplant birthday card, and an accordion folder to house all the paperwork and information we present to our patients and families. We have a decal of our mission and vision statement front and center of our unit, along with a “staff/family portrait” as a reminder to everyone that walks in our department (especially our patients and families), that our goal is for them to feel like they are part of our family. When we tour them on their first day of admission, some of them break down in tears, knowing that we are partnering with them through this journey. Tears and all, our promise remained the same: we will treat them like family. The same goes with their transplant birthday, and on the day of discharge, we sing to them, dance with them, and honor their courage and bravery through this process. Our DH team is not shy about celebrating with them during these pivotal moments of their lives. The Day Hospital team continues to sustain our commitment to providing extraordinary care with love, respect, empathy, and laughter. We successfully implemented our HOPE experience unit-specific co-design, with this, we launch our Comfort Menu. This is well received by patients and staff. A first-time patient at the Day Hospital commented "Day Hospital is a hidden gem of City of Hope".
Care of Self and Colleagues: The Day Hospital team developed an awareness of ensuring that each member of the team gets an uninterrupted meaningful break. The team requested the Leadership team to provide more seating areas at the second-floor outdoor garden of the Pavillion Building, adjacent to the Amini Building. The team developed a Birthday Board to highlight the birthday celebrants on a monthly basis and each celebrant gets an appreciation note from each staff. Also, developed a Recognition board highlighting new staff, newly certified RNs, and staff length of service to City of Hope. A quarterly fun outdoor activity outside of City of Hope is planned by the team based on feedback and consensus. To have ensured the success of enhancing the patient experience required collaboration with Creative Services, Patient Education, Nurse Coordinators, plus the inspiration ignited by the DH UBC Team. Our DH UBC are champions of our initiatives and are inspirations of change. There is never a dull moment when you are part of our DH team, and we are all proud and supportive of each other.
The Day Hospital team is well-deserving of this recognition. The team continues to inspire staff to sustain the mission and vision of Day Hospital. The work and projects of our council are guided by the principles of Relationship Based Care. Mission and vision of Day Hospital: We, the Day Hospital, are committed to care for ourselves so we can give our best to those around. We are committed to care for our colleagues, appreciating the similarities and respecting the differences, always lending a hand. We are committed to care for our patients and family providing extraordinary care with love, respect, empathy, and laughter.
