From a patient’s daughter:
“I would like to thank you for the excellent job that you did as my Dad’s Case Manager. Starting the process of discharge planning early on and advocating for a transitional period at rehab was done tactfully and gracefully. My sister and I were concerned that our parents would object to anything that was remotely associated with long term care, AKA “a nursing home”. We are all very appreciative of your efforts and attention to detail. My father is already doing well and my mother is so relieved to have some time to process all that has happened and plan for the changes at home. I was never really sure what Case Managers did but I now fully appreciate the importance of your role in insuring that all discharge needs for the patient and family members are met. My sister and I will always be grateful that you were there for my Mom and Dad.”
…and she went on to compliment the staff on AC5. “The nursing care was excellent. I did not have one moment of worry or misgivings while Dad was in the hospital. I think it is very important that the nursing staff is acknowledged and given kudos for the high standard of care that they provided my father. I believe it was the nursing care that optimized my father’s outcome.”
