If it wasn't for Destine’s excellent critical thinking skills, assessment, and proper hand-off, there is a possibility that this could have been missed.
I was the nurse who took over for Destine. As I arrived, I could see she was concerned for her patient. She began to share the assessment she conducted. Her patient had a Y90 procedure, and she was to check pulses every 15 minutes. When the patient arrived in recovery, the patient’s pulses went from a plus 2 to a 1. Immediately, Destine escalated to the doctor and to me. I could sense she didn’t want to leave her patient, and she was concerned something might be missed and the patient would return home and have a full blockage of her vessel. I took over the care of this patient, and the pulses went from a plus 1 to Doppler. The doctor did an ultrasound, and in fact, the vessels were blocked, and the patient needed emergency surgery. If it wasn't for Destine’s excellent critical thinking skills, assessment, and proper hand-off, there is a possibility that this could have been missed. Please understand that this is not a one-time event. She constantly rises above the rest with her excellent nursing, intercommunication, and compassion.
