Donna is an incredibly hard-working nurse on the Telemetry unit who also works in Clinical Transformation completing discharge follow-up phone calls. Donna recently identified a patient safety issue related to patient discharge paperwork that is a patient safety issue for all of Bon Secours Virginia.
Donna found during a conversation with a discharged patient that incomplete & out-of-date information was included on the patient's discharge paperwork. Donna immediately set to work to figure out what had happened & what to do to fix the issue. Donna searched through the patient's chart & had to do some serious digging & investigating to figure out what had happened. She eventually was able to figure out the problem, an issue with choosing the correct form among many to print in ConnectCare, and went to work to prevent the error from happening again. Donna used all of the resources at her disposal and has made it her mission to educate the other nurses on her unit as to how to prevent this error from happening again until a more permanent solution can be found. Donna has taken her discovery to the leadership of BSV ConnectCare and presented a reasonable & creative solution to the problem. She advocated strongly on behalf of both patient safety & the ease of workflow for her fellow nurses, both those with years of experience & the new nurse learning their way. Of note, during a conference call with other BSV hospitals it was discovered that the issue that Donna discovered is also occurring at multiple other facilities & further action needs to be taken. I feel that everyone on the call was impressed with Donna's tenacity in pursuit of the issue & her very reasonable & practical solution to the problem.
Donna is incredibly kind & compassionate & is always ready to help with anything she is asked to do. She always has a bright smile on her face & a kind spirit. She gives her all in everything she does & is dedicated to providing the best & safest care possible. She truly is Good Help to those in need!!
