Provider contacted the clinic nurse supervisor and asked if we could see a patient being discharged from the hospital with a new trach and would need dressing changes and trach care 2-3 times a week. This is a procedure that the clinic RN had not done before, and there was hesitation from leadership about whether this was something that could be accommodated.

The patient has had major health issues and has been in and out of the hospital. She declined to have home health follow her after she was discharged. I spoke with Holly and offered some resources that were in the health stream. She researched and became familiar with the procedure and what supplies would be needed for this dressing change. The patient was going home the Friday before Labor Day, and the provider was going to be gone the following week. The patient was scheduled Tuesday morning with both the provider and the clinic nurse. The provider postponed her vacation by 1/2 a day to meet the patient, assess the trach, and do the first dressing change with the nurse. The patient was very appreciative of the accommodations that were made for her to come into the clinic and have the RN do the dressing change 2 times a week.

Her family was able to provide care on the other days of the week. She came into the clinic for more extensive cleansing around the trach tube and to assess for signs of infection along the incision. Holly has been the only RN in the clinic for the past month and has taken on covering in the basket for all three clinics, wound care visits, New Patient onboarding calls, diabetic education, and a variety of other clinical nurse visits. Holly uses the resources available to her through the PMG Nurse Sharepoint site to educate herself and provide evidence-based care to our patients. She conducts herself professionally and with integrity. She collaborates with the wound care nurse to provide the most appropriate care for our patients.
