Jadira has shown so much growth as an RN this past year. She has advanced on the clinical ladder, acts as Charge nurse, preceptor, fills in as resource nurse and is USPIV certified and now applying to further her education and obtain a Master's degree in nursing. I have seen Jadira work through struggles and make positive changes on her unit. She started doing chart audits and giving her peers feedback in an effort to advance on the clinical ladder and support the policies and procedures of UTMB. She is friendly and greets people with a smile always flexible and helpful to others.
Recently, we had a particular patient who is constantly readmitted to the floor due to the patient having ECF with high output causing her malnutrition and dehydration. When this particular patient is admitted she stays for months so the plan of care is pretty much the same throughout her hospital stay. During this patient's last admission there had been many occasions when Jadira cared for this patient, that the patient would asked her questions regarding if there was a chance for surgery to repair ECF as prior surgery failed and ECF returned and the patient appeared to be more down. Jadira was always concerned for this patient and would ask the patient if the team rounded on her and updated her on her plan of care, the patient would say yeah they did but they didn't say. She also would ask the patient, did you ask the team if they can do surgery? The patient stated she didn't ask them. Jadira asked her why? The patient replied I don't know. Jadira quickly realized that the patient got intimidated when a group of MDs would round on her. So, she informed the care manager and resident that the patient has questions about the care plan even after team rounds on the patient. Jadira suggested that not all the team go in at the same time as the patient gets intimidated and doesn't ask questions. After her conversation with CM and the team, during rounds, the team does not go in at the same time to allow the patient to feel more comfortable and ask questions. She would round with this specific patient's team every chance she got to make the patient feel more comfortable. I also informed CM and team that the patient sounded depressed. Psych was consulted and patient was started on an antidepressant. The patient currently still had her ups and down days where she gets "down" as she battled with her chronic ECF but even Jadira is not her nurse for the day, she still stops by to talk to this patient just to lift up her spirits. Jadira exemplifies the qualities of a true DAISY Nurse.
