Jordan has worked as an RN at Children's Hospital Colorado for 4 years. She has been in the Center for Cancer and Blood Disorders for 2 years and in her role as Nurse Care Coordinator on the Solid Tumor Team for 18 months. In that time, she has revamped our Retinoblastorna team. She goes above and beyond in her role.
Our retinoblastoma patients require complex, multidisciplinary inter-institutional care. Providers and nurses from the University of Colorado, PICU, CCBD, transportation, and our RAVE insurance verification team must collaborate to provide safe and high-quality care. Jordan not only identified weaknesses in this workflow, she actively sought ways to improve it. She maintains a running list of active and off therapy retinoblastoma patients. She has organized a monthly multidisciplinary meeting to review this patient population. She created a distribution email list for anyone involved in the care of these patients- easily linking communication between the University of Colorado and Children's Hospital employees. She noted a lack of Beacon chemotherapy plans for intra-arterial and intravitreal chemotherapy. We now have a safe way for chemotherapy certified providers to appropriately order this chemotherapy. She has helped coordinate the flow and transport for these patients who require care at multiple facilities. She has also aligned our retinoblastoma population with the Anchor Center for the Blind. Our team has toured this center and now all bilateral retinoblastoma patients get referrals there. What an amazing alliance for our patients. She currently is working on creating educational materials for this patient population.
The goals of her work have been to improve patient care, increase patient/family satisfaction, and increase provider/RN communication in caring for these patients. She has more than exceeded these goals in her work with the retinoblastoma team.
"As you know, she goes out of her way to attend to every detail in the coordinated care of pediatric retina patients. She has earned trust from peer, physicians, and families. She is totally reliable, totally dedicated and very compassionate. I love having her as part of the retinoblastoma team."
