Krystal Evans took care of an elderly lady with a history of a debilitating stroke leaving her unable to care for herself. Upon discharge, Krystal was unable to get into contact with the responsible party of this patient. She finally contacted a juvenile member of this patient's family who gave Krystal the information to contact the guardian.

After speaking with the guardian, Krystal was very uneasy about sending this patient to the address listed on the conditions of admission after the family requested us to send the patient there. Krystal called the Winter Haven Police Department to do a safety check on the address we had on file. They found a virtually empty house that did not have electricity running to it as well as only one box of food in the pantry.

Krystal called DCF and spoke with the physician to have the patient admitted pending DCF's investigation.

To me, this was above and beyond to make sure that her patient had a safe place with a responsible caregiver.
