Laurie took it upon herself to get the necessary information, made multiple phone calls to get all the stakeholders on the same page.
I would like to nominate Laurie Gray from Care Coordination for the award of the Daisy Award. Laurie deserves recognition for her involvement in a case discharged from the facility in 2019. The patient was to go home with liquid antibiotics for the treatment of their medical condition. Like sometimes happens in discharge, planning the communication breaks down when many are involved. This was the case. All the leg-work Laurie did and through all the education and explanations, the patient went home and was unable to get the liquid antibiotics prescription filled. The pharmacist told the patient/family the medication form needed prior authorization. The Department Manager received a telephone call from the emergency care coordinator and outlined all the issues and how the patient was at home without antibiotics. The next day the Department Manager, in an attempt to resolve the issues, realized some of the data necessary to resolve the problem was missing. Laurie took it upon herself to get the necessary information, made multiple phone calls to get all the stakeholders on the same page. Within minutes, Laurie, on her day off, resolved the issues and had satisfied both the patients and family.
