Our nurse coordinated placement for my mother-in-law in the assisted living, along with emotional support for this transition and patience with my father-in-law. As a family, we have attempted this placement multiple times over the past two years and we are frustrated meeting resistance from my father-in-law. Our nurse worked with Vista Springs and the in-patient clinical staff/providers to have everything set for discharge and to facilitate a smooth transition. On the day of discharge, our father-in-law called stating he canceled Vista Springs and that our mother-in-law was going to Rest Haven, and he wanted her to stay one more night in the hospital. Our nurse tried to explain to our father-in-law that she didn't meet criteria for in-patient care and to explain the miss-information he received from a lawyer. He hung upon her. Despite this, she contacted Rest Haven. She informed them that our mother-in-law was an in-patient (which they did not know) and was able to facilitate a transfer Friday afternoon. My mother-in-law is finally in a safe place with 24-hour care, thanks to her assistance. Our nurse also provided me personal support given the complexity, barriers and last minute changes.
