Jean has been an absolute delight to work with on remote patient monitoring. She has gone above and beyond for one of my patients in particular, including driving to the clinic to help them get their tablet working and meeting with them for an extended period to make sure they felt comfortable using the equipment. Jean's assistance has helped this particular patient stay out of the hospital. Prior to working with her, they were seen in the ER weekly. Since she has been on their team, I have seen a huge improvement. She is kind, thoughtful, understanding, and non-judgmental. Previous people have written this patient off as non-compliant; Jean has taken the time to get to know them and understand how the social determinants of health limit access. She is making a huge difference in their care. I wish all of my patients could meet with her!
