He wanted desperately to make more progress, so she requested a therapy evaluation and managed his progress each day. Myla continued to appeal through Medicaid for his qualification of Complex Medicaid to open additional doors for him.
We had a 32-year-old patient with multiple chronic conditions that include Type I Diabetes, ESRD requiring Hemodialysis and debility. He was hospitalized for longer than a year as the care management team worked diligently to find him long term placement as he did not have insurance. He had been discontinued from therapy services due to his baseline being transfers only. Our Complex Care Manager, Myla Brown RN, BSN, worked extensively with him to assess his personal goals and ensure that once she found placement for him that it would meet his needs. She found placement for him in February 2024 but it was going to find him in a room with four other residents. He wanted desperately to make more progress, so she requested a therapy evaluation and managed his progress each day. She continued to appeal through Medicaid for his qualification of Complex Medicaid to open additional doors for him. She reached out to her leadership team and stated, "I have lost sleep over this and just feel I need to ask but if I am told since we have a bed offer for the quadruple room today I need to move forward with that then of course I will." In just a matter of two weeks, the therapy plan came together for him to qualify for Complex Medicaid. She was able to transfer him to a facility with a younger population and just one person in the room with him. I have worked with Myla for a decade as a team member and as a leader and can attest that this is just one example of what Myla does every day to advocate for her patients. She does not look for the easy solution but tailors each one to her patients but does it with ensuring length of stay management and decreasing their readmission risk. She embodies the spirit of nursing care management in a way that makes our division proud.
