I am honored to nominate Susan Benach, a dedicated and exceptional Registered Nurse Care Manager in the Congestive Heart Failure team, for the DAISY Award. Susan goes above and beyond to advocate for her patients, providing compassionate and personalized care to her panel. Susan's impressive workload is a testament to her dedication and expertise. She works closely with the care team to ensure seamless coordination of care, navigating complex clinical situations with ease. Her ability to provide additional assistance to providers has been invaluable to our team.
Notably, Susan serves as the central hub for cardiac transplant patients, expertly guiding them through the intricate and emotionally challenging transplant process. This involves coordinating with multidisciplinary teams, managing complex medication regimens, and providing emotional support to patients and their families. The transplant process is notoriously complex, involving Comprehensive evaluation and testing, including cardiac biopsies and imaging studies; Detailed review of medical history, social support, and psychological factors; Coordinating with organ procurement organizations and transplant surgeons; and managing post-transplant care, including immunosuppression and complication management.
An example of Susan's exceptional care and teamwork was highlighted by a peer, who shared the following kudos: "I wanted to give a kudos to Susan Benach. A patient who is currently enrolled in home-based cardiac rehab came to the cardiology clinic one day asking for someone to help him with additional notes/paperwork that he needed for his return to work. Susan could have easily asked me to take over or told the patient that he wasn't one of hers, but she did it! She reassured him that whatever he needed would be completed and even followed up with his employer to make sure the documents were received. Susan is such a team player in this department. I am grateful for her, and I know our patients are as well."
Susan's remarkable 40+ year nursing career is a testament to her enduring passion for delivering exceptional patient care. As she prepares for her well-deserved retirement in April 2025, it is fitting that we recognize her outstanding contributions to our team and the lives of countless patients.
