Chanel's patient was a little forgetful, and we wanted his wife to also hear the discharge instructions. The wife decided she wasn't coming up to floor to get him and she told staff to bring him down to her in the lobby. As it went, the wife didn't get out of the car! Chanel went into the car to give instructions. Chanel realized that the meds were different on the W10 AVS than what the patient was used to so she had to call the Doctor to get things changed and more accurate. She spoke to him twice. I also went into the car to me sure info was accurate for the patient and that the patient and wife were comfortable with new plan of visiting services and obtaining an appointment with his endocrinologist the very next morning. Later on that day, Chanel talked to the discharging MD on the phone with how he could update the paperwork in Epic. She spent almost an hour with the patient and his wife in the car, and another 45 min with the doctor via phone. She smiled, and was professional and genuinely supportive the whole time. She is a true nurse, and went above and beyond for this patient and doctor!
