I would like to nominate Tatyanna Coleman from Mercy Place as a deserving recipient of the DAISY Award. This registered nurse should receive the award for her clinical skills, compassionate care, exemplary service and for her continued commitment to excellence.

The following are specific examples of the nominees’ extraordinary commitment to patient-centered care:

Tatyannna Coleman is an outstanding nurse practitioner in the Community Program Department. She is very conscientious, understanding, and helpful as she provides compassionate care in the area of chronic disease management for uninsured patients at SJMO Mercy Place Clinic in Pontiac MI.

In addition to seeing patients at the clinic, she oversees a diabetes telemonitoring program where she provides individualized case management and one-on-one coaching via telephone and/or computer interactions on a daily basis.

She also participates extensively with community outreach activities where she provides screening and education to patients with high blood pressure, diabetes, and high cholesterol in the underserved community.

Tatyanna is a native of Pontiac and holds the community near and dear to her heart. She is committed to the well being of her community and her quest for making the community healthier.
