Nurse Traci's job entails care coordination and everything else a nurse does for a multi-disciplinary Mental Health team, which is a lot. Nurse Traci is very good at her job because she cares about each and every Veteran that comes through our doors. Many times, she is the first person I call when I can see that a Veteran is having a hard time. Nurse Traci was asked to help a Primary Care Provider's Veteran get connected with services. She never met the Veteran face-to-face, she has only spoken with him and his wife over the phone. She helped him set up a desperately needed intake for a prescribing provider in a different Mental Health Department, and then the Veteran no-showed his appointment, twice. Even after Nurse Traci called him the day before to make sure he was still okay to come both times. Nurse Traci reached out to the Veteran to see what the holdup was and was able to get out of him that his anxiety and fear were too high for him to feel comfortable meeting someone new, in person, following a long travel time to get to American Lake. After speaking to the Veteran's wife about their concerns and their hardships with a new intake appointment, Nurse Traci was able to identify a prescribing provider who would be a good fit for the Veteran's dire needs and personality conflicts. She even arranged with this provider to schedule an intake outside of their normal intake times. Most people would call it a wrap at this point and consider a job well done, but not Nurse Traci. Nurse Traci knew that this Veteran needed to be seen and a plan developed to help him take charge of his mental health. Nurse Traci worked with the Veteran's Primary Care Provider to prescribe a medication to help him relax before coming to his newly, 3rd time, rescheduled intake appointment.
