I am writing to nominate Ann Hampton RN, Utilization Denials management, Laura Johnson, CVICU Pharmacist, Cindy Wolford APRN and Glynda Meadows RN CVICU manager for the DAISY Team Award to honor their extraordinary teamwork and dedication to patient care. Their collective efforts on a Sunday prove their professionalism, compassion, and commitment to excellence. The patient, upon discharge, was prescribed Eliquis 5mg. This patient is unable to drive and attempted to fill his prescription. While most of his prescriptions were covered, he learned that his Eliquis, essential for preventing stroke, was not covered. This caused immediate concern, as this patient had not taken this medication for three days since discharge putting him at a significant risk. Driven by the urgency of the situation, Ann called the discharging unit, hoping to find a way to receive the medication. This was unsuccessful, so Ann then contacted Glynda Meadows, to brainstorm on how to best help. After receiving the information, Glynda reached out to Laura Johnson, a highly skilled CVICU pharmacist and Cindy Wolford APRN. Together, they exemplified exceptional teamwork and patient-centered care. Despite it being a Sunday, all parties sprang into action without hesitation. Laura stated she would be in CVICU that afternoon and would work with the providers to get the necessary medication. She recommended contacting Cindy to see if she was able to assist. When Cindy was notified, she immediately asked, “How soon can you meet me at the hospital?" Cindy coordinated the logistics and brought the medication to WHH Publix pharmacy, and Ann drove to the hospital, picked up the medication, and delivered it to the patient at home. On a Sunday afternoon. Thanks to the collaborative efforts of Ann, Laura, Cindy, and Glynda, this patient received this crucial medication promptly, thereby mitigating potential adverse outcomes. Their willingness to work beyond their regular hours to secure the best possible care for a patient in need is truly commendable.
The actions of Ann, Laura, Cindy, and Glynda go above and beyond the call of duty and showcase the essence of extraordinary nursing and healthcare leadership. Their exceptional teamwork prevented harm and exemplified the highest standards of patient advocacy and care.
